Click on the link below to read and complete the It’s Your Life waiver. Once submitted, you will be provided a link to proceed to our wellness portal to complete your WebMD Health Assessment.
If you participated in It’s Your Life last year, please use the username and password that you created. If you are unsure of either, please click on “forgot username or password”.
If this is your first year participating in the It’s Your Life program or your name has changed, click on “Register” to create your username and password for this site. When you register, your registration ID is your first and last name as written on the form you submitted to the It’s Your Life office followed by your 8-digit date of birth.
